LVCCH provides nursing support to promote the health, safety, and well-being of individuals receiving residential services.

Nursing services are provided based on each individual’s assessed needs, Individual Support Plan (ISP), and healthcare provider orders and may include:

• Nursing assessments and health monitoring
• Medication monitoring and clinical oversight
• Coordination of medical care and follow-up
• Monitoring of acute and chronic health conditions
• Staff training on individual-specific health needs
• Physician-ordered nursing treatments and interventions
• Review of hospital and emergency department visits
• Communication with healthcare providers, families, and the individual’s support team

Services are provided within the nurse’s professional scope of practice and in accordance with the individual’s authorized plan of care and applicable Pennsylvania requirements.
